For which complaints are Nasal Aesthetic Operations (Rhinoplasty) used?
Rhinoplasty surgery is a procedure for fixing the shape of the nose, removing the obliquities and giving the nose a more proper shape fit to the facial structure.
Through aesthetic nasal operation; the size, obliquity, arc of the nose can be fixed, large nasal holes can be made smaller, the shape of the tip of the nose can be changed, the angles between the nose, forehead and lip can be fixed. Also, congenital or traumatic shape impairments can be fixed, breathing problems can be removed. Most of the people with visible obliquities also have obliquities inside the nose, and this may cause breathing problems. It is difficult to fix outer obliquities without fixing the ones inside. The nose is a whole structure and both breathing problems and aesthetic faults are solved with this approach.
In which age can Nasal Aesthetic Operations be done?
The new shape of your nose will be planned according to your facial anatomy, thus it is preferred to postpone the surgery for young people until they complete their development. This age is around 16 for girls, a little later for boys. Before the Operation
Communication is the principle between you and your doctor and it is very important. Your surgeon will ask you how you want your nose to look and what you want in the first meeting; examine the shape of your nose and face and tell you the best possibilities for you. The structure of your nasal bones and cartilages, shape of your face, thickness of your skin, your age and expectations are the primary topics of your meeting. Digital planning will let you have an idea and it will let you express yourself more easily.
The first thing to do when deciding on an aesthetic nasal surgery is to search for an aesthetic surgeon whom you will trust and believe in. It is important because aesthetic nasal operations give very well results when done properly, and they cause damage difficult to take back when done improperly. Here hand skills and experience of surgeon are very important. During this visit and examination, shape of the nose, skin, cartilage and bone structure, functional status will be evaluated. Detailed information will be told about possible results after the operation.
Tests before the surgery, anesthesia and operation technique that will be used, their risks, costs and other options if present will all be explained. Most of the health insurance policies do not cover this surgery, which totally has aesthetic purposes. However, if the surgery will be applied due to breathing problems or for correction of clear shape impairment after a trauma, insurance may cover the surgery.
You must be sure that you told any past nasal surgery or treatments even the ones years ago, allergy and breathing problems, drugs you use and smoking status before the procedure. Planning and Computer Work:
Technological possibilities of our day provides us major benefits. By taking pictures of the face and loading to a computer, we can work on it to show you how your nose will look after the operation and its compliance with other structures of the face. Also, during this work, we can understand patients’ expectations and decide whether they are realistic or not.
All these procedures require experience. Other possible operations can be discussed during this interview: mentoplasty (aesthetics of the tip of the jaw), lip thickening or operations of the cheek.
If there are inconsistencies between the nose and face, interventions to other areas of the face make the results of the nasal operation better. The most common one is the jaw being in the back according to nose and forehead. If this is the case, through an operation to the jaw, profile look of the face can be made much better. We diagnose this condition in approximately %30 of our patients, and enlarge the tip of the jaw with tissue injections.
The planning of nasal operation is important for doctor and patients to get to know each other. During this interview, surgeon will provide information about the technique, anesthesia, surgery location, possible results and post-operative process.
Anesthesia, Operation Type and Place
I have been doing aesthetic nasal operations under general anesthesia, which I believe is better for myself and the patient because it is more comfortable, excluding some minor interventions.
| Like all my aesthetic surgical interventions, I perform aesthetic nasal surgeries in hospital environment, sterile operation room conditions. Depending on the duration of the surgery, patients are usually discharged on the same day of the surgery. |
Operation can be done in either closed or open techniques. In closed technique, all the cuts are made inside the nose and there are no scars outside. In open technique, a small cut is made onto nasal columella additional to inside cuts. Surgeon’s experience and preference is important for decision of the technique. I use closed rhinoplasty technique in almost all of my cases. However, for secondary operations or nasal aesthetics after trauma, I prefer open technique.
The operation usually takes 1-2 hours depending on the majority of the deformity.
For patients who do not have serious breathing problems, small non-cohesive wound care materials are used after the operation. These materials can be removed easily on the second or third day. Rarely, special silicone splints are used. Thus, patients do not have troubles of tampons which they usually fear of.
Post-Operative Process and Return to the Normal Life
Slight swelling and empurpling is normal after the operation, especially in the eye contour, pain is not common. Intermittent cold application for the first 24 – 48 hours reduces the swelling significantly. Also, some cream and sun protective creams helps empurpling and fastens the recovery. These complaints occur the next day of the surgery and begin fading on the third day. Every passing day your nose will look better and your morale will be raised. However, recovery is a stepped process.
You can return to your daily activities after a day or two. Usually, a control visit is planned on the second day and the 7th. At the end of a week, plaster casts are removed and a thin band is placed on the nose, in order to reduce the swelling. After two weeks, band is also removed and nose is exposed, depending on the clinical condition. In these days, swelling will fade away to an unnoticeable level.
At the end of the third week, you will see your nose is much better than the old one and receive positive feedbacks.
The decision for return to work may depend on patient’s job, social status, swelling and discomfort level. Usually, patient can return to work after two weeks but it is possible to work after a week when preferred. It is more comfortable to return to work and social life when plaster cast is removed, by help of cosmetic products.
Aesthetic nasal surgery is difficult but common and efficient when done properly. Your possible breathing problems are also solved with this surgery.
It provides very good results when done properly. However, improper surgeries may cause bad results that are difficult to undo. Therefore, choosing the surgeon is very important for this surgery. Hand skills are very important for a surgeon. On good hands, complications are rare and minor. These rare complications are; bleeding, infection and post-operative deformities.
The nose is always better than before with a surgery performed by a surgeon with good skills. Although, due to difficult anatomy of the nose and unexpected healing problems, aesthetic nasal operations may not always reach wanted levels and there is a possibility of need for a second operation in %5. This rate is considered normal in world standards. This procedure is a minor make up depending on the condition of the deformity. The duration of this intervention and recovery is very short.
